E-Filing Services

WELCOME TO DOLE-NCR e-FILING SERVICES!

DOLE-NCR e-Filing service is a free, online process for the submission of applications and reports and other labor-related programs and services. This free service powered by Google Forms allows clients to register, fill out forms free of charge and submit applications and monitoring reports, request necessary permits, certifications and documents and perform a number of other interactions with DOLE-NCR in a secure online environment. Furthermore, this free service was developed in support of the minimization of contact rate recommended under the DTI and DOLE Interim Guidelines on Workplace Prevention and Control of Covid-19.

This e-Filing service is accessible to ALL clients that can provide a valid email address which is required for access for the submission of applications and reports. Additionally, some of the programs at e-filing service that requires uploading of accomplished forms and other required documents may require user to sign in to Google Account.

13th Month Pay Report
13th Month Pay Compliance Report Every covered employers are required to submit a Report on Compliance with PD 851 (13th Month Pay Law) to the nearest DOLE Regional Office not later than January 15 of the following year. The report on compliance should include the name of establishment, address, principal product or business, total employment, number of workers benefited, amount granted per employee, total amount of benefits granted; and name, position and telephone number of person providing the information.

Request for Certificate of Involuntary Separation for SSS
All employees including Kasambahay and Overseas Filipino Workers (OFWs) who are covered for the grant of unemployment insurance or involuntary separation benefit shall apply for DOLE Certification of involuntary separation as contemplated under SSS Circular No. 2009-011.

Registration of Establishment under Rule 1020
Every employer as defined in Rule 1020 of OSH Standards shall register his business with the Regional Labor Office or authorized representative having jurisdiction thereof to form part of a databank of all covered establishments.

Registration of Job, Service Contractor, Subcontractor (D.O. 174-17)
All persons or entities acting as contractors are mandated to register under D.O. 174 with the Regional Office of the Department of Labor and Employment (DOLE) where it principally operates.

Construction Safety and Health Program Application (Simplified)
Approved Construction Safety and Health Program (Simplified) is primary requisite for having an approved building permit for two-storey below (construction of residential building) or minor repair works with less than 15 workers.

Construction Safety and Health Program Application (Comprehensive)
Before the start of the actual construction, the construction project manager shall prepare and submit to DOLE Regional Office a comprehensive construction safety and health program for approval or modification.

Request for Technical Safety Inspection
(Renewal of Permit to Operate and/or Certificate of Electrical Wiring Installation)
No mechanical equipments (boilers, pressure vessels, ICEs, elevators, hoisting equipments and similar equipment and/or electric wiring) shall be installed and/or operated in the Philippines without the permit and/or certificate issued for the purpose by the Secretary of Labor and Employment or her duly authorized representative. Application for installation of mechanical equipment/s and/or electrical wiring/s shall be filed with the Regional Office with available professional mechanical and/or electrical engineer for processing and verification.

Annual Medical Report
Pursuant to OSH Standards and Department Order No. 198-18 Implementing Rules and Regulations (IRR) of Republic Act No. 11058 “An Act Strengthening Compliance with Occupational Safety and Health Standards and Providing Penalties for Violations Thereof”, employers, contractors or subcontractors, if any, shall submit to DOLE all safety and health reports and notifications such as but not limited the Annual Medical Report(AMR), OSH Committee Report, Employer’s Work Accident/Injury Report (WAIR), and Annual Work Accident/Injury Exposure Data Report (AEDR).

Report of Safety Organization
Pursuant to OSH Standards and Department Order No. 198-18 Implementing Rules and Regulations (IRR) of Republic Act No. 11058 “An Act Strengthening Compliance with Occupational Safety and Health Standards and Providing Penalties for Violations Thereof”, employers, contractors or subcontractors, if any, shall submit to DOLE all safety and health reports, notifications and organizational plans and minutes relative to the implementation of OSH Program.

Company OSH Program

The employer shall submit a copy of the Company OSH Program to the DOLE Regional, Provincial, Fieldor Satellite Office having jurisdiction over the workplace. The employer shall ensure that the core elements of OSH program are integrated in the company OSH program such as management commitment and employee involvement, workplace risk assessment, hazard prevention and control, safety and health training and education, and OSH program evaluation. A duly signed company commitment to comply OSH requirements together with the company OSH program using the prescribed template shall be considered approved upon submission EXCEPT for Construction Safety and Health Program which shall need approval by DOLE prior to construction.


Minutes of the Meeting of Safety and Health

Pursuant to OSH Standards and Department Order No. 198-18 Implementing Rules and Regulations (IRR) of Republic Act No. 11058 “An Act Strengthening Compliance with Occupational Safety and Health Standards and Providing Penalties for Violations Thereof”, employers, contractors or subcontractors, if any, shall submit to DOLE all safety and health reports, notifications and organizational plans and minutes relative to the implementation of OSH Program.


Working Child Permit

The employer, parent or guardian shall first secure Working Child Permit (WCP) from the DOLE before engaging the services of any child below 15 years of age allowed under Republic Act No. 9231 (An Act Providing for the Elimination of the Worst Forms of Child Labor and Affording Stronger Protection for the Working Child, Amending for This Purpose Republic Act No. 7610, as Amended, Otherwise Known as the “Special Protection of Children from Child Abuse, Exploitation and Discrimination Act”)


Application for DOLE Clearance (Certificate of No Pending Case)
Through the exercise of visitorial powers under Article 128 of the Labor Code, as amended, the Department is mandated to review labor standards cases coming from Regional Offices and Order of the POEA Administrator pertaining to recruitment violations.
This DOLE Clearance Certificate of No Pending Case is issued to stakeholders for bidding purposes, renewal of license and withdrawal of escrow deposit.

Application for DOLE Clearance (Certificate of No Pending Case on Child Labor)
Through the exercise of visitorial powers under Article 128 of the Labor Code, as amended, the Department is mandated to review labor standards cases coming from Regional Offices and Order of the POEA Administrator pertaining to recruitment violations.
This DOLE Clearance Certificate of No Pending Case on Child Labor is issued to business establishments that maintains a company policy expressly prohibiting child labor and not have a pending case with the Department on the use of child labor.
